Such individuals are allowed to submit identity or address certificates duly attested by a Gazetted Officer, Councillor of the respective Municipal Committee, Member of the Legislative Assembly, or Member of Parliament.<\/p>\n<p>The current format of the application has been inducted by the Income Tax Department to regulate the procedure.<\/p>\n<h2>Procedure To Apply For Verification Certificate<\/h2>\n<p>The format for Verification Certificate is mentioned<em><strong>\u00a0under provisions of rule 114(4)\u00a0<\/strong><\/em>of Income Tax Rules, 1962 of PAN (individual\/HUF) and it is available for download at the IT department\u2019s official website.<\/p>\n<p>An applicant can\u00a0<em><strong>download the Gazetted Officer form for <\/strong><a href=\"https:\/\/www.antworksmoney.com\/blog\/pan-card-status\/\"><strong>PAN\u00a0<\/strong>card<\/a><\/em> in fillable Pdf version and fill in the required details. The application has two sections &#8211; one for the applicant and the other for the corroborator.<\/p>\n<h3>One Has To Fill In The Form With The Following Information for Verification Certificate<\/h3>\n<ul>\n<li><strong>For the applicant<\/strong>\u00a0\u2013 a passport size photograph (same as the one affixed on\u00a0PAN application\u00a0for), applicant\u2019s name, father\u2019s name, date of birth, address of residence (in case the applicant has resided at more than one address in the past one year, they have to mention it), office address, and if the applicant has their name changed then they have to mention the previous name.<\/li>\n<li><strong>For the corroborator<\/strong>\u00a0\u2013 full name, designation at work, the name of the organisation or department where they hold the position, office address, employee id number, photocopy of the I-Card, contact number, and signature with an official seal.<\/li>\n<\/ul>\n<p>Also, there are two other certificate that can be used in an applicant does not have identity or address proofs. These are:<\/p>\n<h3>New Gazetted Officer Certificate for PAN Card (Annexure A)<\/h3>\n<p>This form is to be filled by the Gazetted Officer, MLA, MP or Municipal Councillor. On the right hand corner of the form, the applicant has to affix a passport size photo and then read the keep in mind the following details to know how to fill Annexure A for PAN Card.<\/p>\n<ul>\n<li>Father\u2019s Name<\/li>\n<li>Date of Birth<\/li>\n<li>Resident Address<\/li>\n<li>Office Address<\/li>\n<li>Name Change (in case of any change in name)<\/li>\n<\/ul>\n<p>The issuing authority is required to fill in a declaration that affirms their role as the certifier of the applicant. Then, they have to attest the photograph of the applicant with their signature and official rubber stamp. Both signature and stamp have to appear half on the photo and half on the application form. The issuing officer also needs to fill in the form with the following details:<\/p>\n<ul>\n<li>Signature of the Issuer<\/li>\n<li>Full Name<\/li>\n<li>Designation<\/li>\n<li>Department\/ Organisation\/ Constituency<\/li>\n<li>ID Card number along with a photocopy of the card enclosed with the form<\/li>\n<li>Telephone Number<\/li>\n<li>Mobile Number<\/li>\n<li>Address with location<\/li>\n<li>Official Seal<\/li>\n<li>Date<\/li>\n<li>Place<\/li>\n<\/ul>\n<h2>Certificate Of The Employer (Annexure B)<\/h2>\n<p>The use of Annexure B in PAN Card is the almost the same as Annexure A. But in this case, the applicant can only submit Annexure B if they do not have an address proof.
